If you lose or forget the password you set when you initially visited the survey website, you can email it yourself again by following these steps:
Once you are within the online survey (i.e. from the main survey menu page onwards), you are able to print a copy of your survey, including any responses you have made, by clicking on print in the left-hand menu. This will bring up a new window with a printable version of the survey - go to file and then print in order to print it out from your PC.
